When a member’s home becomes a barrier to their health, you don’t have to solve it alone.

You make the referral. We handle the rest. Through our partnership with Anthem Blue Cross, eligible Medi-Cal members may qualify for ramps, grab bars, railings and accessible bathrooms.

Kinda chic to spend three days at CAHP with people who get it💚

Three days in Palm Desert, connecting with healthcare professionals, building relationships and celebrating 40 years of the California Association of Health Plans.

We even rolled up our sleeves to support the No Child Hungry cause with ModifyHealth helping pack 1,000 bags for families across Fresno and Madera counties.
